In partnership with your hospice, our clinical services help you improve patient care outcomes and elevate your staffs level of clinical knowledge by providing patient-specific drug therapy expertise while assisting you with the critical tasks of lowering your overall medication costs and increasing the quality of patient care. In addition, our programs are designed to support the patient medication profile review requirements and data collection efforts of the new Medicare Conditions of Participation (CoPs). Several options are available and our clinical staff will work with you to develop the program that best meets the needs of your hospice while optimizing your local resources.

HospiLogic™ 

With HospiLogic™, you’ll receive automated patient assessments generated by our proprietary clinical software which includes more than 20,000 hospice-specific clinical rules. All you do is enter the patient’s information and our system does the work, checking for effectiveness of therapy, adverse effects, interactions, duplicate therapy and possible drug therapy alternatives. An electronic report of the patient assessment is available on demand whenever you need it. HospiLogic’s patient assessments are a great tool to assist you in achieving compliance with CoPs. 

HospiMTM™ 

HospiScript’s clinical pharmacists retrospectively review your patient drug profiles (entered by the hospice via our online patient eligibility portal, HospiDirect™) and provide you with a written consultation. Reviews are provided upon admission and within required time frames thereafter. Additional reviews are available when requested for medication changes, symptom management, etc. Patient drug profiles submitted from the hospice via fax or other electronic interface can also be accommodated. 

HospiOnDemand™ 

Real-time, prospective drug utilization review conducted telephonically by a HospiScript pharmacist, provides documentation to the hospice for each consultation upon admission and at least every 15 days thereafter. Consultations are available anytime as requested for medication changes, symptom management, etc. With HospiOnDemand™, you can rest assured that our highly skilled hospice pharmacists are there when you and your patients need them most.
