Pharmacist on Demand (POD)
HospiScript is pleased to announce that Phyllis A. Grauer, R.Ph. Pharm.D. CGP and Palliative Care Consulting Group (PCCG) have joined the HospiScript Services Team. This partnership provides clients with access to expanded clinical and educational services including on-demand pharmacy consulting services. Having ready access to clinical pharmacists with palliative care expertise gives frontline nurses, physicians and the entire interdisciplinary team confidence that they are appropriately, consistently, and cost effectively managing their patients' medications.
The HospiScript "Pharmacist on Demand" Program provides clinical consultations based on a fee-for-service structure. Patient-specific, patient care-focused consultation is provided to your nurses or other hospice clinicians by clinical pharmacists who specialize in hospice and palliative care.
As a POD customer, you'll be provided with clinical pharmacist consultations for each of your patients upon admission. Consultations for symptom management issues are also provided on an "as needed" basis.
Your clinical pharmacist will collaborate with your staff to:
- Determine covered and non-covered medications based on the patient's diagnosis
- Suggest cost-effective therapy alternatives
- Address duplicate therapy and polypharmacy
- Avoid potential drug-drug interaction and side effects
- Discuss rationale and provide communication tips for drug therapy recommendations
- Participate, as desired, in the prior authorization approval process of non-formulary medications
Following each POD consultation, the clinical pharmacist will document all recommendations and communicate the covered/non-covered medications to the HospiScript Customer Service Team. The patient profile and medication changes are system supported within thirty minutes.
